Cypripedium parviflorum, or Small Yellow Lady's Slipper is native to the northeastern quarter of the US, and can also be found in several Western states. It is a perennial that grows in moist, open woods, on woodland edges, as well as in swamps and bogs, and sometimes along roadsides and railroads, in appropriate conditions. It blooms in late spring and early summer with small yellow flowers.
